software bugs happen, i just dont get how they dont have a separate monitoring process in place to trigger the killswitch, especially since it's part of your core business. If you need to wait until the exchange to tells you something is messed up, that's a fail right there.
let it run for 30mins is inexcusable, very curious what the delay was, doubt we will ever know though.
