Underreporting is for mild side effects, for adverse reactions, your own source says - We also know that underreporting is less common for more severe adverse reactions than for those that are more mild.

And death being a very severe adverse reaction, it completely destroys your narrative that there are tens and thousands of more cases that are not reported
