People don't understand how it works. A lot of US nuclear assets are mobile and undetectable. One SSBN can come to near-surface, not get the "we're ok" signal and barbecue certain countries on a pre-determined plan. Its a dead-man switch. Enemies know it and that is why it doesn't happen.

Just a minor skirmish, mainly a air campaign Iran's pretty big though, civilian casualtys directly will be low, until you factor in all the deaths from lack of medicines, drinking water, food and power then that'll be 1Mil+ like in Iraq.
If Iran got a missile off odds are USA would intercept it anyway doubt they've got more than a few if any so not a concern at all.
