Chain of command for this is the president--> SECDEF--> CG CENTCOM. GEN Votel is CG CENTCOM. Prior to that he commanded USSOCOM and before that, JSOC. He's an unassuming, experienced professional and the furthest thing from a war monger -- the last thing he wants to do is start WWIII and if he's ordered to do something stupid, he has the integrity (unlike many GOs) to publicly step down. Mattis won't try to micro manage him and Trump delegates to the max which means Votel will be able do his job based on what's actually happening on the ground, if a military response is ordered.
If any Russians are injured or killed it will almost certainly be their fault. Putin knows it and even Trump knows it because we don't just launch military strikes on a whim. The president is thoroughly briefed on multiple courses of action and their advantages and disadvantages, and he ultimately chooses one or sets parameters. And before we actually strike, it's standard protocol for our military to give the Russian military ample warning but not enough to compromise the mission. The people who do these handshakes are calm, professional and friendly -- despite media and social media hysterics.

I shall keep the bathtub thing in mind however while I think an incident is bordering likely, not a nuclear one. Those of us who grew up in the cold war have strong memories of living teenage years with that extra element of hey, we may die at any moment to add to the nihilism.
