This is probably related to "food combining", i.e., if you're going to eat carbs, limit them to fat-free or virtually fat-free carbs such as fruit, some cereals, etc. The idea is that there be little or no fat there when the insulin kicks in. Then wait two hours before eating anything containing fat.
How this works out in practice is something like strawberries and shredded wheat for breakfast, then a regular lunch and dinner. Bread might qualify if it had no fat in it, but you'd have to eat it without, of course, butter.
