If I understand Simmons/Pozzi - they are suggesting that there may be a rupture/leak in the well casing itself several hundred feet below the seafloor and away from the riser. The riser being shown to the public at large (live feed "Top Kill" zone) is small relative to the potential casing/well head rupture some miles away.
