I'd have to say the person who has to pump portable toilets with that vacuum pump.

I knew a guy that worked in an old folks home, he talked about giving a guy an enema and having the guy do a projectile diarrhea stream across the room. You do that job without a protective diving suit so maybe it's up there with the sewer diving?
