Because humans are so close to pigs genetically speaking, the virus can make the leap only by jumping from birds (usually ducks) to swine, and then to humans from swine.
It's been this way for as long as vaccines were attempted - the epidemiologists always travel to Asia, and usually mainland China, to test the poultry population for strains of influenza, so as to be able to create an egg-based culture for a human influenza vaccine for that season.

The ducks infect the pigs, the virus modifies its genetic 'code,' and it makes the species jump from the hog to the human; something it found much, much more difficult to do when it primarily affected the duck.
Avian influenzas main barrier to infecting humans en masse is the inability to sustain human to human genetic transmission ability.
