is there an actual market for CFDs or are CFD firms just bucket shops?

But they did the next worse thing that is, they made me put up a three-point margin and compelled me to pay a premium at first of a half point, then a point, and finally, a point and a half.
This is probably a either a really dumb question on my part or one of those English/American translation issues, but do you mind explaining what that means?

That is a quote from ROASO.
Livermore is saying the Bucket shop, instead of banning him outright, upped his margin requirements and also widened the spread on him. one and half points wider than normal, or maybe even 3 points wider if he had to pay the premium both on entry and on exit.
