With the new TWS version (921) and "autoconfirm v6.1 x64 and v6.1 x32" (the versions compiled with Autoit) there was a problem due probably to a small change in how fast the confirmation button was available for firing the the auto-click.
I think I fixed it by adding a delay of 50 ms to the auto-click. ( barely noticeable to the human eye)
Let me know if it works on your systems, here now it works fine.
The last version compiled with Autohotkey (only for 32 bit systems) still worked fine here, thus I didn't change it.

I'm using 927.3 and I'm not able to get rid of the confirmation message. There simply is nothing to check the way it is described...
"Support for single-click order transmission has been reinstated in the BookTrader. This ability had been deprecated in favor of an order confirmation message. Customers can now elect to turn off the Order Confirmation by selecting "Don't show this popup dialog again" when they click in the ladder to create an order. Once the order confirmation message is turned off, clicking in the price ladder will instantaneously submit an order at the specified price."
