I don't see any particular error in your window (except the login problem) , but if that's all you have on that window, there's def something wrong in your "installation\setting", mine is at least twice longer.
When you use IB controller with a "regular "account (no demo) and the password generator is needed, you need to insert manually the generated code and then it should work( I haven't tried this because I 've used it only on a demo account).
However here is my IBcontroller folder, it's set only to get rid of order conf. window, there's not passsword and user name set in the ini file, you can launch sampleIBControllerStart and enter them manually when asked or edit and save the ini file.
Find this lines in the ini file and edit accordingly:
IbLoginId= yourLoginId
IbPassword= your IbPassword
1)Make sure your TWS is installed in C:\ directory
2) extract the zipped folder
3)Copy and Paste the extracted folder in C:\ directory (first move your Ibcontroller folder in another directory in case it's already in C:\
It works perfectly here, just tested
