I think you're getting distracted by the questions of "who" and "why" -- "what" is the question on which to focus.

You're correct that there are many fake orders, but those are simply another part of the DOM landscape that one must understand in order to interpret the action. Similar to the way that a basketball player might look one direction and step another, you need to read the body language and balance of the market and recognize the "head fakes" for what they are.

There will always be headscratchers that don't seem to fit in to the larger picture you've constructed. However, keep in mind that even traders of large size make mistakes too.
