Even those who can't afford it.... The U.S. facilities will be happy to give you a loan and put you on a payment plan.
People don't want to wait 18+ months for basic medical procedures.
If they have good credit
People without insurance in The US wait longer than that.
Oh -- I thought Obamacare would guarantee they had insurance.

Republican Governors blocked obamacare medicaid expansion from millions of Americans but I'm sure you knew that.
Obamacare was a flawed concept and a give-away to lobbyists from the day it was passed, but I’m sure you knew that.
With Max E. his focus is on things that have cost him money or not. His perspective on Dodd-Frank is that his US broker was not able to give him the excessive leverage he needed for his diversified system anymore so he went to some fly-by-night abroad and got ripped off.
Therefore Dodd-Frank is/was thoroughly bad and corrupt.
It is about a short-sighted and silly as you get but hey, he has to blend in with the locals now.
