Well considering the outcome is the same no matter what, it becomes a question of how will you live your life as the lever puller? If seeing the trolley do a sick loop will bring me joy, then certainly the best choice for me in that moment would be that one. If I would rather embrace the pain of five people dying sooner rather than later and just get on with life beyond that, then that's the best choice. So there's really no right or wrong answer. It's simply a choice to make based upon one's preference for living.
 
Assuming there is no one in the trolley, I would pull the lever . Flipping the lever might result in some mechanical difficulty between the the lever and the trolley. The trolley is thrown of its path and nobody dies.
 
Either don't touch the lever, don't get involved, you can't be implicated then, or 2nd choice, flick the lever when the tram is halfway through the points and attempt to derail it.
 
In both cases the same number of people.
But in one case you let people die, in the other you actively choose to kill one set of people over the other.
