Being uninsured in America will cost you more

my understanding is, the irs can only collect the aca penalty by deducting it from a refund, pretty easy to avoid getting a refund, especially if you pay estimated taxes each quarter. doesn't make sense, but what about this law, or the irs in general ever does?
