Bad or missed execution?

How can a limit order be placed with X number of shares on the DOM and have all the orders filled plus say 5 or 6 thousand more orders filled before the price moves upward without your order being filled?
limit orders being placed on active trader pro via smart routing seem to just sit and sit and sit while time and sales shows fills way beyond the original DOM.
I am new and maybe missing something but this happens continually and am missing fills.
Your help would be greatly appreciated.
 
Limit order to go long, sits at the bid, your then in a cue, first come first served, sellers sell to you, price is just moving up before it fills your order

Can you not see what position your order is with your dom?
 
Could you attach printscreen from time and sales?

Maybe some ecns cross the markets or it was just delayed fill on dark pools.
 
Thanks for the replies. Here is a copy of the time and sales, my limit order was placed at 10:04 and the same number of shares showed on DOM. As you can see in the T&S many orders filled ahead of mine at 7.82 or it appears that way.

Maarketmonkey - all depends on which ECN you have put your order.

Notice that at .82 bid - orders were filled at DF, Q, DEX. So if you put your order on ECN that is not filling at this moment - you will not open position. I.e. you send pedning order on BATS - but on BID were filled only on EDGX, NASDAQ oraz some dark pools.
 
Thanks, I thought they crossed for best bid/time, this clears things up.

I retired in the Philippines and have been trading the Philippine exchange where they have one ECN, its like fish in a barrel, I guess I am the fish now.
