Advice on DDQ

Hi, I have a prospective institutional investor who asks for DDQ. What is the normal procedure here, to have it completed by a 3rd party, or as self-assessment? Should it be supplied by the investor itself or are there standard versions available that are universally acknowledged by the finance community? It's my first biggie, so I don't want to appear as a newbie here... :-)
