When hurled at successful people its usually hurled by jealous people with an inflated opinion of their own self worth wondering why that person who achieved something seems confident fulfilled and charismatic.
Obama, Woods, other athletes... These guys have achieved... they kicked ass. What is not for them to like about themselves. They get to walk on a different plane of existence. They nurtured their talent, executed a game plan and kicked ass.
In my opinion - if you are top 100 in the world in something... you have a right to think you are better than the rest. If you are president of the United States. You kicked every one of our asses in realizing our stated 4th grade goal. .... why would I expect him or her to live like an average person.
For instance do we really think Tiger woods goes to therapy thinking he is all screwed up? should he feel that way?
next time someone calls a succesful person a narcissicist - wonder if he or she is part of the "get something for nothing from the government crowd." you will soon see part of the invidious pattern. I am seeing it happening in school to one of my sons. he does well in a few things... but he does not want to stand out so his friends like him.

Actually true clinical Narcisicism is a mental disorder. The Doc in this case says that he a real Narcissist.
One may wonder, well how does this guy know? He has never met him. Well, as you know the FBI analyses all kinds of psychopaths from affair. You don't have to meet them to know it.
Read this guys resume. He is not the type to put stuff out that he doesn't believe. His reputation is too important for that.
