Therein lies the issue. Those who can afford it don't care. Those who can't afford it suffer. I suspect there are many more Americans who can't afford it than can afford it.

Perhaps, culturally, this is where America is different from that of other countries. The willingness to have a little less in order to give a fellow citizen in need a little more.

I live in Canada and am free to choose any doctor I like... Want a second opinion? No problem. I've never had any real major medical issues so I'm not entirely sure how the system works in that regard (major surgeries etc), but in terms of general health care, I can absolutely choose my doctor.
