ameritrade Level 1 size is fake

That's right. using the TOS platform, and all their others asfaik , they only show the bid ask size for the primary exchange, not the total of all routes. So they can be showing 100 when its really 10,000 You would need to look at level 2 to get the right figure. Inexcusable. IB does it...

    Future Trading with Interactive Brokers

These days, unless you're talking an event, or some random spike 4 ticks (1 point) is rarely found in a short timespan. So maybe this is some lame system that only trade when there's a big spike move, and expects to get a good fill when it's impossible.

Does other brokerage firms in USA charge money for not enough transection per month like IB does?

I don't like paying fees but it's really not an inactivity fee at all. It's a fee for the data, which technically is NOT free, but is fee waived if you trade a certain amount. A platform like TWS for $12 a month is nearly free. Years ago , garbage with 1/100 the features was $350 a month

    DeMuth bagged thousands of traders

So this guy Chris Demuth over at seeking alpha, was pimping the RAD 'arb' for a year, up until the day it blew up. He has many followers. Lots of folks lost good money on this. Don't think he ever issued a sell. Charges $2500 a year . I'm sure he has winners, but this was an amateur show.
