    To answer your immediate question: I am not familiar enough -- when I have shopped, I have paid more, but the sample size is quite small -- once a quarter for 5-6 years? So, I saw no reason to change, but based on loose data. Back to your OP, you'll find a better market on 'fence post' strikes...

    Think of the distance (and the dollar-pain you self-inflict to get there) as the deductible portion of your health/car/fire insurance. If you're that far OTM, even if you have "lots" in insurance, it may not do you a bit of good. If you really "need" the hedge, you want it activated ASAP.

    However long you've been trading via a certain set of rules/procedures/protocols, count on those working only maybe for another quarter. Have other irons in the fire, and compare performance monthly if not weekly. The markets lending you good performance right now WILL change, and will go away...

    My own spreads' width varied with time and distance from the market, shrinking as δ↑ or Θ↓. I'm not sure of your question, but surmise this advice worthwhile to offer: hedging event risk with any manner of farOTM options is generally ill-advised.

    No. Rebalancing is needed when a fund's constituent parts are out-of-balance [!!] with its intended portfolio weights. This may happen because XYZ has fallen, or simply because ABC has climbed.
