Nah, problem with that is you don't know if it came from trading, selling courses, or selling crack in street corners. Anybody selling you trading service/courses..just as for that form. See how they react. People are fine with making crazy claims, but as soon as you ask for the irrefutable...

Can I ask question, how do you separate your trades out like that, especially with a few positions entered way before. When I traded multiple positions on TV, my positions gets liquidated in order of entry/exit (FIFO). What platform are you using?
