I agree. I think they overlooked it in the example.

So is this example then incorrect: https://www.interactivebrokers.com/en/index.php?f=713&nhf=T ? The fixed cost is always $150K for 30Mil share sale. It does not add the (0.0000218 * Value of Aggregate Sales) transaction fee or the (0.000119 * #shares) FINRA fee. The Tiered column adds them.
