Hard to say without more facts about the way you trade personally. For low costs, I'd point you towards a broker like CedarFX because they have no commission and decent spreads. Trade245 and Oanda are also good choices, so it may help to compare the 3 and see which you like best.
I think that depends. Some losses are inevitable and you shouldn't sit around obsessing over them. If you're taking a ton of losses, however, it would be necessary to investigate.
